What do I do if I create a duplicate account or input wrong information during Self-Onboarding?

Please note that only one responsible individual should register the account for the facility during Self-Onboarding. If multiple employees should have access to the dashboard, the account owner (the one who registers the account) can invite them through the dashboard. Please find the instructions here on how to invite someone to the dashboard : Here
 
Please make sure to double-check all information before registering your facility during Self-Onboarding. 
 
If you have created a duplicate account or have entered wrong information by accident, please submit a ticket on HelpDesk
